Do external framerate limiters add lag only when Vsync is on or when both on and off?

When I get high FPS, around 400ish my character model seems to "jump" around when moving. It looks like a few frames are missing.

Hi, So I want to set a FPS cap for League of Legends because the highest you can set it to in game is 80 and if I leave it uncapped I get 400-500 FPS which is way too high.
