Monitor is disconnecting and reconnecting randomly

I have a BenQ XL2411Z, using Windows 7 64-bit, and whenever I wake my computer up from sleep mode, I will see my desktop for about 4 seconds, then the screen goes black, windows plays that sound like when you unplug a USB device, and then after 2 seconds the monitor turns back on and everything is fine until the next time I wake my computer up from sleep mode.

What causes this?
