If you wanted to try whatever he did, probably something like below.

Driver paths:
C:\Windows\System32\drivers\mouclass.sys (has digital signature)
C:\Windows\System32\drivers\mouhid.sys (doesn't have digital signature)

Right click the file (mouclass.sys) -> Go to digital signature tab -> click on ...

Mousetester by default locks to 2 specific cores, what you're seeing is likely the impacts of reprioritization of resources due to other processes/drivers on those cores. I'm assuming if you change the mousetester affinity (through task manager) you'll get other interferences.

A powershell script to compare which drivers and services load in normal mode vs safemode, optionally it has a guided function that asks you if you want to disable each driver from the differential. Make a registry back up before using if attempting to disable services.

This is an issue that likely only affects AMD. Task manager should show the cache numbers for your CPU, not sure about AMD but Intel will show different caches sizes either e-cores on vs off. E-cores have an additional cache that get enabled/visible in windows when they are enabled.

Can you see if you're getting any significant debounce latency? Some keyboards won't go lower then ~22ms on this test because of it (razer huntsman elite), the newer huntsman elite v2 can change this setting.

Yea for sure! Older games typically have lower latency since the overhead is nothing on modern gaming systems so small changes perhaps more apparent?
