My guess would be you're using nVidia Gsync incorrectly or AMD's FreeSync incorrectly. It's worth asking if this just started?

Ah, then going by that, you're probably able to consistently exceed (or at least near reach) your max refresh rate pretty comfortably, depending on in-game setting priorities. Here is ULMB and I will circle back to follow Chiefs instructions on the others. This was taking with the default iPhone camera settings after I enabled ULMB. Does this seem good or bad?
