Obviously something is defective. If there's no warranty anymore and can't RMA it, then a workaround would be to use GPU scaling instead of display scaling. You can force that in the GPU driver control panel.

It doesn't matter, but usually you want something only very slightly above 60 (like 60.01.) In this case, you don't need RTSS for the cap because you can just use the nvidia driver limiter with a cap of 60. I was thinking that lower will be better for games that have in-game 60fps cap that can't be...

I tried FPS cap with RTSS and Nvidia driver, but this alone didn't change much at @120Hz. Game runs perfect with in-game cap and vsync @60hz, but @120Hz judders, however with 1/2 Nvidia vsync it's perfect again. Now the question which set-up has higher input lag: 1. Is is higher if the game's cap is higher than monitor Hz / 2. You should still do the RTSS cap I recommended in post #2.

Reboot, close all applications that might be starting with Windows (Discord, Skype, Afterburner/RTSS, RGB lighting control software, etc.) Only leave one tab open in the browser.
