Hey,
I got the Optix MAG241C which is 144hz monitor i mainly play Fortnite but i have this in EVERY game.
As soon as i go under 120fps, (119) i start lagging insanely bad this includes:
-Input lagg, if i move my mouse.
-Stutter lagg, its hard to move around everything is choppy and stuttery
NO screentearing at all.
I tried it on my old pc, and now on my new pc (i7k, 1080ti, 16gbram)
Could i have a setting wrong in nVidia cp? or what could this be?

Are you using VSYNC OFF or are you enabling FreeSync (G-SYNC Compatible)?
Bigger frametime is always more input lag.
50fps means 1/50sec GPU rendering lag
100fps means 1/100sec GPU rendering lag
150fps means 1/150sec GPU rendering lag
It's unavoidable. Getting a faster GPU helps.
Now for stutter issues, you want to enable FreeSync or GSYNC (but make sure to cap about ~3fps below max Hz). Eliminating the stutter can help aiming, though it depends on how bad the stutters are -- if the stutters are very bad, the use of VRR can help aiming. Also, are you using an SSD in your new computer?

I got an 1080ti which is like the highest possible expect a few gpus.
Windows & Fortnite both installed on SSD.

VSYNC OFF doesn't prevent frametime lag (GPU rendering latency). Higher framerates on a single GPU always has lower lag.
You may need to also test different monitors to see if they may help your latency issues. A 240Hz monitor will typically have significant lower lag, given the 240Hz Fortnite Survey. This gets expensive though, but it's additional data points worth noting.

I play Fortnite if i enable VSync i literally feel the delay whenever i press a key.
My problem is that as soon as i go under 120 (Even 119) my game gets sooooo unsmooth it feels like 40 fps.

Are you using an FPS cap of 120? I don't see why "120" is a magic number here otherwise. If the game feels fine at 180FPS, 130FPS, 120FPS but not at 119FPS, then... well that doesn't make any sense whatsoever.

-Unlimited, its fine
-160 (its fine, no screantearing)
I never cap at 120, but on ANY fpscap below 120 feels horrible
