[240HZ] G-SYNC ON - VSYNC OFF
LLM "ULTRA"
If there are 150 frames on a 240HZ monitor and the GPU 99%, is there no effect?
Should the 240FPS frame be higher?

I assume you're talking about Nvidia's low latency mode right?
Well, changing the setting won't actually effect fps, but it does effect latency, though it can introduce other problems like CPU based stuttering because the CPU would have to process the frames right when GPU has finished, and that may cause stutter problems with games that use lots of CPU to begin with, e.g. RTS games with lots of units etc... because the CPU would have to be ripped away from it's other tasks in order to process the frame it just got from the GPU.

Yes. It only useful if your GPU usage is 99%. And you will get less latency if you just lock your FPS at the value where you don't reach 99% GPU usage.

Watch this video where the guy clearly demonstrates it that you want to turn it off and just never reach 99% GPU usage:
