Find the folder where the 0000 subfolder has a lot of keys in it. In the 0000 folder add:
"PowerMizerEnable"=dword:00000001

I’ve used MSI Afterburner for years and I’m pretty sure that applying the settings at Windows startup just means that MSI Afterburner will launch minimized/in the background. I think the GPU behavior will go back to normal as soon as the program is closed. If not, there’s still a service running or something. As far as I know, without editing the BIOS, there’s no way to save these settings to the GPU.

No, it does not work. I have it set globally and per game too. I also have ultimate performance mode in Windows. The GPU is still downclocking and fluctuating however it wants to, even going as low as 300 MHz when nothing is running.

It doesn't pin it to max 24/7, it just pins it to max when it is using it. And Windows power settings aren't GPU setting either.
Your problem may be something else.
