Image blurry at 3840x2160 but not 4096x2160?

Image blurry at 3840x2160 but not 4096x2160?

So I'm trying to get my 4k 60hz TV to work without success. I can use 4096x2160 at any refresh rate and the image looks great, as it should. However, when I'm using 3840x2160, the image gets really blurry at anything over 23hz. I simply want to get 3840x2160 to work but have no idea why it's presented to blurry. Ideas?
