Interesting. I'm going to clean install 1909 and see if it fixes the issues I've been having. I noticed another odd bug, sometimes when I alt tab from a game (mainly Destiny 2) back to windows, freesync or something messes up and Windows gets stuttering as well (monitor shows around 86Hz, when usually it's at 240Hz.) I have to turn the monitor off and back on for it to register as 240Hz.
Considering doing another clean install of Windows and see if it fixes it, or possibly install 1909 instead of 2004..

guys, do you see 280hz in windows adapter settings in vg259qm or only 240hz? When I turn on overclock mode at 280hz i can only see 240hz? But in application softmccs i can see 1920*1080@280hz? I can play valorant only at 240hz, it forces maximum refresh rate always even if you have it in windows on 144hz for example.

If in NVIDIA Control Panel, scroll down, sometimes there's multiple groups of refresh rates for a specific resolution. One set under "Ultra HD, HD, SD" section (where 60Hz is) and one under "PC" section (where 144 Hz is), for the 1080p resolution.
It's confusing sometimes.
Blur Busters even has an article, "Oh No, I’m At The Wrong Refresh Rate: How To Change Refresh Rate" for those people stuck at 60Hz...

What do you mean which screen? I have AMD R9 290x. I can only get 240hz, even tho I overclock it to 280hz/270hz with OSD. I tryed reinstalling drivers, pluging in/out display port, the cable is new with monitor. The thing is I can still see it when I change from 240/270/280, it creates new resolution in CRU, but it is not showing it windows display adapter nor working ingame. I tryed creating custom res (copy pasted CRU) but it is not allowing me to create new resolution in AMD drivers. https://prnt.sc/u5gvt0

Maybe the pixel clock of the AMD R9 290x doesn't go high enough for above 240hz
