For latency, "highest" is always best as far as I know. From a smoothness perspective, "highest" is also best when using VRR (freesync/g-sync). Without VRR, matching FPS and Hz is best for smoothness.

When i use Highest i Play on 240hz With only roundabout 190-200 fps. Wouldnt that be stuterring af? I currently use 200 fps and Turn down refresh Rate to 144 hz

With VRR active, it doesn't stutter. Without VRR, yes, it will stutter. But in this case it's called "judder", to differentiate between the stutter of FPS/Hz mismatch and the stutter created by the game itself.
