Because it's not only your pc that's sending updates to the server, it's all of the players. So the server has to run at full speed all of the time, hence the limits on the tickrate. Everything that happens inbetween ticks is handled by the interpolation in the game.

Fortnite "Creative Mode"

Nvidia Control Panel Settings:

G-SYNC ON + V-SYNC ON, full screen mode, limit in-game 237 fps "stable 237" = MORE SMOOTHER
G-SYNC OFF + V-SYNC OFF, full screen mode, limit in-game 240 fps "stable 240" = LESS SMOOTHER

Its possible or placebo?

Why would it be placebo... of course its going to feel smoother with Gsync on...

at the same frame shouldn't both be equally smooth?

It doesn't work like that just because you are capping it at 240fps.

Gsync vs sync off is basically smoothness and no tearing at the expense of a bit more input lag and a change in mouse feel.

Why do you think the latter scenario would be equally as smooth as the former? It won't be, because it doesn't align to the scanout of the display, hence tearing, which is inherently less smooth. It's as simple as that.

If you want "smooth" G-SYNC off + V-SYNC off with a framerate at the refresh rate, you need to use RTSS Scanline Sync, but, unlike G-SYNC, it's a fixed refresh solution, and thus effectively breaks if you can't keep the framerate at the refresh rate at all times and GPU usage at a certain percentage.

G-SYNC is the lowest lag "set and forget" no-tear frame delivery solution available.

That "input lag" and "change in mouse feel" is directly due to a lack of tearing with G-SYNC, and if the framerate is to adhere to the scanout, it will always be limited to the max refresh rate of the display, so at currently achievable refresh rates, G-SYNC off + V-SYNC off will always potentially have lower lag directly due to tearing, especially with framerate sustained far above the refresh rate (where multiple tearlines can begin to appear per scanout cycle).

I am a huge g-sync fan and on 240 hz its a beauty because it allows you to reach those high FPS rates that most pros look for, usually g-sync on 120 hz or 144 hz monitors is tough because for it to work you shouldn't exceed the monitor's refresh rate which means your FPS is limited to 120/144, but at 240 hz it's a perfect scenario in my opinion.

But I just asked Poach and Chap and seems that both do not use g-sync, which makes me question whether they don't use it because its a bad feature cos of the input lag or because they just aren't that familiar with it.

Some people probably use G-sync (or half the time, or a small percentage of time) and don't know it (Basically how 99% of the gamers 'use' G-sync, they don't set it up properly so its not active most of the time)

Lets just say that most pro gamers just setup their sens, keybinds, resolution and be done with it, until or unless they encounter an obvious problem. Most just blindly aim for maximum possible FPS, and don't consider that smoothness could have some advantages as well for a few ms increased latency.

At the end of the day its all personal preference and most configuration differences are probably affecting end results for a few % at best.

Maybe they know that chasing those percentages of benefits could quickly lead to 'tweaking' madness that a lot of the tweaking community (A lot of childish communities that grow like crops these days) get themselves into. Consequently I think that most people chasing the few % of benefit are just average or good players that can't get any better by practice and think that min maxing their configuration and build can get them over the edge to that pro competitive scene.
