New Settings:
1.) All network card options (Device Manager) disabled

a) Interrupt Moderation = On
b) Interrupt Moderation Rate = Off
d) Network card Transmit Buffer (Device Manager) = 2048
e) Network card Receive Buffer (Device Manager) = 2048

2.) REG: Computer\HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows NT\CurrentVersion\Multimedia\SystemProfile

a) NetworkThrottlingIndex = 70 (Decimal)
b) SystemResponsiveness = 10 (Decimal)

3.) CMD@Admin: netsh interface tcp set global rsc=disabled

4.) CMD@Admin: netsh interface udp set global uro=disabled

5.) CMD@Admin: netsh interface udp set global uso=disabled

6.) CMD@Admin: bcdedit /set tscsyncpolicy enhanced

7.) CMD@Admin: bcdedit /set disabledynamictick no
Now the TTK and TTD feel normal.

It just saddens me that I could play at 5-6ms, but it doesn't run smoothly, even though the FPS is good. The mouse also feels weird.

bcdedit /set useplatformtick yes

EDIT:

NetworkThrottlingIndex = 70 (Decimal)

I'd like to understand why it feels so terrible at, say, 10 or ffffffff? Everyone recommends it, but it doesn't work well for me.

I forgot to mention two things.

1.) I disabled virtualization in the BIOS.
2.) I disabled the BCLK TSC HW FIXUP option in the BIOS.

I can currently play very fairly. No advantages. No disadvantages. It feels right.

A very common recommendation in many performance enhancement/gaming guides state that disabling NetworkThrottleIndex improves network performance and latency because in theory it should prevent rate limiting and quality of service (QoS) interactions.

The main purpose of NetworkThrottlingIndex is to reduce (rate limit) calls which would otherwise impact real time audio and perhaps cause stutter or other audible artifacts.

Disabling it may increase throughput, but it does not improve DPC latency which is probably more beneficial for lower latency applications such as video games.

Regarding all bcdedit tweaks , I wouldn't touch any of that, unless your ACPI QPC RTC timers( run on Cpu-z ) are in sync.

I played with the current settings all day yesterday (including during prime time), and it worked fine. I'm not looking for an advantage, but I don't want a disadvantage either. The settings work. I haven't been able to play properly for weeks.

Are you sure its the software and not your isp being poo poo from time to time?

Mine acts normal for some time and then this happens for the next 2 weeks
