Looks dead to me, I would have wondered about GPU problems but you've tested it with a laptop too so it's not your desktop gpu.
you shouldn't get banding like that even on the factory default settings.

You *MUST* use the supplied Dual Link DVI-d cable when you get one back, and not VGA, if you want to use 120hz or 144hz.
VGA is analog and subject to interference at high rez/refresh and just doesn't have the bandwidth. I'm not even sure it would let you try.

Item has been RMA'd i dispatched it back to scan yesterday.. damn it feels so weird playing with 75hz 16ms again now, i literally don't think i want to play any games until i get a replacement
it pains me though that this will most likely take a few weeks to replace being the christmas period and all.. I think the process is they actually have to receive the faulty item, go through tests.. and then once a decision is made send a replacement.

I just got mine today and I am having a similar issue. I made a thread, and I'm hoping I don't have to send it back. I'm actually quite upset (not mad, the other upset, the one where I feel terrible and legitimately sad), I hope I don't have to send this back to Amazon. =/
