Look man, I already went down this rabbit hole. I really don't think it is ISP problem. You are just gonna end up on a dead end and waste more money changing something. Here, have changed ISPs 3 times, diffrent routers/modems, mobile hotspot and bunch of other things. Not much difference. And get this, problem exists different locations across continents. Same issues. Different ISPs and network infrastructures.

I live in one of the most developed urban centers in the world. I use a very advanced and sophisticated ISP (it already had FTTH in 2007), but there is still no improvement. Changing ISP, modem, and router did not help.

The thing is, it really feels like an internet problem. But then it should be verifiable. I can run speed tests, packet loss tests, ping plotters, etc. all day, but the problem isn't visible. 15ms ping. 1-2ms jitter. Others play with 100ms and have no problems. They get 100 kills per round. No lag.
My neighbor has a VDSL 250 from O2. Same problems in "The Finals" and "Escape from Tarkov." Always lagging. Always desyncing (it feels like it). Previously, 1&1, then Telekom, then O2. Never a change. Always terrible.
That's why you shouldn't be mad at the people who shout "ISP." It really feels that way.

In my case the problems started after changing ADSL to FTTH so yes the ISP is the culprit in many cases

Can you measure it? Or is it just a coincidence that one game isn't working, but everything else is perfect?

15.) 1x FritzBox 6670 Cable = Wait and see (but I think terrible)

17.) Exit lag = Sometimes good, sometimes terrible (if good = good without)
My problem:
1.) VDSL line belongs to Telekom. No matter the provider = Always Telekom infrastructure (terrible)
2.) DOCSIS line belongs to Vodafone. No matter the provider = Always Vodafone infrastructure (terrible)
3.) No third line available from other providers
4.) 4G/5G is always overloaded, according to people here
5.) Rented apartment
I don't need snake oil. I need a solution.
Try running off-grid . Inverter and dc batteries.
And which country are you in ?

I noticed something, but I'm not 100% sure if it's directly related to my gaming problem.
When I get killed, I notice two things:
1.) I'm lying there and I see the enemies. They're moving completely normally.
2.) I'm lying there and I see the enemies. They're "trembling" and their movements are slightly stuttering.
If point 1, then: Good hitreg.
If point 2, then: Broken hitreg.
